Generic treatment options

The core muscles external to the pelvis may need addressing, e.g., paraspinal, gluteal and anterior abdominal wall muscles. The adductors and iliopsoas also need to be assessed and managed. Within the pelvis muscles such as coccygeus, iliococcygeus and pub-ococcygeus rectalis may have trigger points, as may the piriformis and obturator internus muscles which span from the pelvis to the hip. The ischiocavernosus and bulbocavernosus may also be involved.

Symptoms And Signs

Musculoskeletal pain typically presents as a dull aching pain that is difficult to localize. Structures including the rectus muscle, iliopsoas, quadratrus lumborum, pir-iformis, and obturators, innervated via T12-L4, and levator ani, innervated via S2-4, can refer pain to the lower abdomen pelvis and vulva or vagina. This pain is further enhanced if there is a primary pelvic pathology or irritation, including bladder and colon activity, menses, and intercourse, as these organs also have a similar innervation of T10-S4,10,108,109111 therefore, myofascial pain is often worse during menses. On digital examination of the abdominal, back, or vaginal dermatomes, pressure on a trigger point evokes local and referred pain. Performing a straight leg raising maneuver tensing the abdominal wall muscles exacerbates abdominal wall pain.

Neurolytic Intercostal Nerve Block

Thoracic paravertebral nerve at each level gives anterior and posterior branches. The anterior branch is the intercostal nerve which gives lateral and anterior cutaneous branches. They provide motor supply to abdominal wall muscles via T7-T11 and muscles of the forearm and hand via T1. They provide sensory innervation of the skin of the chest and abdomen anterolaterally. The intercostal nerves travel in the inferior part of the ribs posterior to the intercostal artery. Subcostal nerve comes from the ventral ramus of T12 and is equivalent to the intercostal nerve at the above levels.

Ataxia is often considered a consequence of central drug action. However, it should be realized that a disturbance of muscle coordination may result from a drug action upon muscle tone. The reduced muscle tone can be easily measured by the resistance to fingers pressed gently into the abdomen. Even though this examination is very subjective, when performed by an experienced observer it is a reliable and sensitive tool to differentiate between drugs that cause ataxia by muscle relaxation from those in which ataxiogenic action resides centrally. Therefore, when removing animals from the open field, abdominal muscle tone can be evaluated by palpation and rated according to the scoring system described by L scher and H nack 0, normal muscle tone 1, equivocal muscle relaxation 2, unequivocally reduced muscle tone and 3, markedly reduced muscle tone.18

In 12 patients, this confirmed the lack of involvement of the quadriceps muscle. In very advanced stages, however, only the vastus lateralis is truly spared. Computer tomography (CT) of the calf muscles demonstrates that the soleus is much more affected than the gastrocnemius. Paraspinal and anterior abdominal muscles are clearly affected too.
