Drug Actions on the Skins Glands

Acne is a common glandular problem arising from hyperproliferative closure of individual glands in the unique set of pilosebaceous glands located in and around the face and across the upper back. Both local and systemic antibiotics and antiseptics suppress the formation of lesions. It is believed these attenuate the population of anaerobic microorganisms that are deep seated in the gland, the metabolic byproducts of which irritate the lining of the gland, setting off lesion formation. Mild cases of acne improve and clear under the influence of astringents, possibly for the same reason. Retinoids, oral and topical, reset the processes of epidermal proliferation and differentiation. Through such dramatic influences on cell growth patterns, they actually prevent the formation of lesions. However, because of concerns over toxicity, they tend to be used only in the most severe cases of acne and thus are prescribed for those patients whose acne lesions progress to cysts.

Menstrual Irregularities and Bipolar Disorder

PCOS is one of the most common endocrine disorders occurring in reproductive-aged endocrine disorder with an estimated prevalence of between 4 and 6 38 . It is characterized by chronic anovulation and hyperandrogenism 39 . Chronic ano-vulation can lead to menstrual abnormalities and infertility. Hyperandrogenism can manifest as hirsutism (excess hair growth on the face), acne, and male-pattern balding. Metabolic consequences of PCOS include obesity and IR, which may lead to type-2 diabetes mellitus and cardiovascular disease (CVD) 39-42 , and underscores the relationship between reproductive abnormalities and IR in women. PCOS is associated with overweight or obesity in approximately 50 of women with the disorder 43 . Even if they are normal weight, individuals with PCOS tend to have a higher proportion of abdominal body fat 44, 45 , which is a risk factor for both IR 46 and CVD 47 . Lipid abnormalities have also been found at high rates among women with PCOS, including low...

Aminolevulinic Acid Photodynamic Therapy

In addition to skin cancers, ALA-PDT has also been used to treat skin conditions such as acne vulgaris, warts, condylomata, scleroderma, nevus sebaceous and psoriasis, as well as nondermatological applications such as carcinomas of the digestive system, respiratory system and urogenital system 22 .

Role of Pharmacotherapy

Androgens play an important role in physiologic aspects of the female sexual response. However, the effect of androgen therapy on sexual function in women is controversial. Some studies have reported improvements in libido, sexual arousal, and the frequency of sexual fantasies with testosterone therapy in a variety of forms. The observation that testosterone therapy may result in improvement in mood and well-being is felt by some researchers to be most important. The central sex ster-oid's effect on mood may be what underlies sexual function in both women and men. Potential side effects of androgens include a decline in serum high-density lipoprotein (HDL) cholesterol with oral preparations and mild cosmetic side effect such as hirsutism and acne. Testosterone preparations include creams, gels, and tablets that can be taken orally or used sublingually. They are not approved in the US FDA, but one product approved in Europe for postmenopausal women is a transdermal testosterone patch....

Chemical Structure Of Oxytetracycline

Oxytetracycline Molecule

Tetracycline hydrochloride is also available in ointments for topical and ophthalmic administration. A topical solution is used for the management of acne vulgaris. Meclocycline, tetracycline sulfosalicylate (Meclan), is a semisynthetic derivative prepared from oxy tetracycline.184 Although meclocycline has been used in Europe for many years, it became available only relatively recently in the United States for a single therapeutic indication, the treatment of acne. It is available as the sulfosalicylate salt in a 1 cream.

Taurine Interaction With Guanfacine

For nerve block anesthesia, 250 ophthalmic uses, 1112 pharmacological properties, 247 for spinal anesthesia, 251 for topical anesthesia, 248 Tetracycline(s), 762-766 absorption, 762 for acne vulgaris, 1083 adverse effects, 765-766 antimicrobial activity, 762 chemistry, 763 distribution, 762 dosage, 764 excretion, 762-763 for H. pylori infection, 631 hypersensitivity reactions, 766 interactions

Decrease in Side Effects

CD have also demonstrated their potential to reduce the irritation caused by drugs. In the same way that inclusion of a guest inside the cavity of a CD can decrease its attack by the surrounding elements, it can also protect the surrounding medium for undesirable side effects of the host molecule. An interesting application is the decrease in the ulcerous effect of nonsteroidal anti-inflammatory drugs administered by the oral route. This effect has a double origin local and systemic. The protection that can be conferred by inclusion in a CD can only be local and, furthermore, is incomplete, because of the equilibrium existing between the dissolved complex and the free active ingredients and empty CD. However, inclusion of phenylbutazone 98 or naproxen 99 in P-CD or of indomethacin in P- or hydroxylpropyl-P-CD 100 results at least in a decrease in the ulcerous effects on the gastrointestinal tract. The reduction of corrosive activity of drug molecules on ocular tissues 50, 101 and skin...

Toxicology testing

In addition, if there is no data on toxicological effects of an NCE in pregnant animals, then it will never be given to pregnant humans, even if the risk benefit ratio is expected to weigh heavily toward using the new medicine. Teratogenesis is essentially defined as the abnormal growth of a foetus. The thalidomide disaster which occurred 40 years ago horrifically proved that the placenta was not a perfect protective barrier. With only one exception, every drug that has been since shown to be teratogenic in humans has been shown to be teratogenic in animals (4). Adequate reproductive toxicology testing in animals cannot guarantee safety in pregnant women, but it is the best available and with the anti acne treatment isotretinoin it has prevented a disaster of the magnitude of thalidomide where a foetus had a 20-30 risk of malformation.

Other Biosurfactants

Although there is an increasing potential for the application of biosurfactants in the biomedical field, some of these molecules may constitute a risk for humans. For instance, P. aeruginosa is a bacterium responsible for severe nosocomial infections, life-threatening infections in im-munocompromised persons and chronic infections in cystic fibrosis patients thus rhamnolipids have to be well-investigated prior to such uses. P. aeruginosa strain's virulence depends on a large number of cell-associated and extracellular factors.102-104 Cell-to-cell signalling systems control the expression and allow a coordinated, cell-density-dependent production of many extracellular virulence factors. The possible role of cell-to-cell signalling in the pathogenesis of P. aeruginosa infections and a rationale for targeting cell-to-cell signalling systems in the development of new therapeutic approaches was discussed by Van Delden and Iglewski.102 Synthesis of rhamnolipids is regulated by a very...

Chapter Overview

The medicinal chemistry of vitamins is fundamental not only to the therapeutics of nutritional problems but also to the understanding of the biochemical actions of other medicinal agents that directly or indirectly affect the metabolic functions of vitamins and coenzymes. Notable in this regard is the interrelationship between folic acid and vitamin B12 in the development of megaloblastic anemia, as well as the efficacy of warfarin in relation to vitamin K status. In addition, vitamins may rarely be used in treatments unrelated to their role as a vitamin. The use of retinoic acid in acne, niacin in hyperlipidemias, and hydroxocobalamin in the treatment of cyanide poisoning are a few that are discussed within this chapter. Accordingly, this chapter includes a brief summary of the history, chemistry, kinetics, biochemistry, therapeutic use, adverse effects (hypervitaminosis), and available vitamin products that can assist the clinician in a better understanding of the role vitamins play...

Therapeutic Uses

Vitamin A is used in the treatment of known or suspected vitamin A deficiency. Vitamin A deficiency in developed countries such as the United States is not likely of dietary origin but is more likely caused by malabsorption as is seen in sprue, hepatic cirrhosis, biliary, or pancreatic diseases. Interestingly, vitamin A levels decrease during infection with the measles virus (rubeola) resulting in increased morbidity and mortality, especially in children with preexisting vitamin A deficiency. Vitamin A supplementation has been shown to decrease mortality by 50 and prevent blindness in children developing measles if given at the time of diagnosis. Of course, vaccination is of prime importance however, in underdeveloped countries where the rate of vaccination is low, vitamin A supplementation is a beneficial alternative.41 Retinoate analogs have been developed for use in the treatment of dermatological conditions such as psoriasis and acne. Some therapies for cancer by retinoate analogs...

The most common adverse drug reactions are diarrhea, rash, acne, pruritus, dry skin, nausea, vomiting, and anorexia. A higher rate of most of these adverse events is observed in patients treated with 500 mg day of gefitinib as compared to treatment with 250 mg day. Most adverse events are of mild-to-moderate grade. Less than 2 of patients have permanently discontinued therapy. Adverse drug reactions usually occur within the first month of therapy and generally are reversible. Asymptomatic increases in liver transaminases have been observed and periodic liver function testing should be performed. Interstitial lung disease, which may be acute in onset, has been observed uncommonly in patients receiving gefitinib, and some cases have been fatal. The overall frequency of interstitial lung disease is 0.3 outside of Japan and 2 in Japan.

Vitamins Vitamin A

Retinoids have been used as pharmacologic agents to treat disorders of the skin. Psoriasis, acne, and rosacea have been treated with natural or synthetic retinoids. Moreover, retinoids are effective in treating symptoms associated with congenital keratinization disorder syndromes. Therapeutic effects stem from its antineoplastic activity (Brzezinska-Wcislo et al. 2004). Patients suffering from these illnesses may be supplementing with vitamin A and their dosages should be explored.
