Although cross-tolerance may occur among antipsychotic drugs, clinical problems occur in making rapid changes from high doses of one type of agent to another. Sedation, hypotension, and other autonomic effects or acute extrapyramidal reactions can result. Clinical worsening of psychotic symptoms is particularly likely after rapid discontinuation of clozapine and is difficult to control with alternative antipsychotics.
