The plantar digital nerves are derived from the posterior tibial nerve and provide sensation to most of the plantar surface.
In a sterile fashion a 25 g 1.5 in. needle is inserted proximal to the metatarsal head adjacent to the bone of the toe to be blocked. The needle is advanced from the dorsal surface to the plantar surface since the plantar digital nerve is on the dorsal side of the flexor retinaculum. A total of 3 cc of local is injected after negative aspiration.
