Saphenous Nerve Block at the Knee

To perform this block, the patient is placed in the lateral position with the operative leg slightly flexed. In an aseptic fashion, a 25 g 1.5 in. needle is slowly advanced through a point in front of the posterior ridge of the medial condyle, toward the medial condyle of the femur. At a depth of 0.25-0.5 in., a paresthesia is sometimes obtained in which the needle should be slightly withdrawn. In the absence of persistent paresthesia and after negative aspiration a total of 5 cc of local is injected.
