Paliperidone is a second-generation antipsychotic medication indicated for the acute and maintenance treatment of schizophrenia. It acts as a dopamine D2 and serotonin 5-HT2A antagonist. It is the 9-hydroxymetabolite of risperidone with which it shares many pharmacological properties. An extended release formulation of paliperidone allows for once-daily dosing, results in lower peak plasma levels and hence less side effects, and thereby limits the need for dose-titration that is otherwise required at the start of treatment with risperidone.
