Lofexidine is as an agonist at a2 adrenoceptors and acts presynaptically to decrease adrenergic neurotransmission in the central nervous system, notably activity of the locus coeruleus. Through this mechanism, it decreases many of the autonomic symptoms of opioid withdrawal in humans and in animal models. It is under investigation for managing withdrawal from heroin or ► methadone and has been found more effective than placebo. Its efficacy appears similar to that of clonidine, another a2 agonist, but it has lesser adverse effects.
